(Note: All student-athletes must have health and accident insurance. If your family does not have medical insurance, you may purchase school-sponsored health insurance.)

Complete a sports physical through your family physician or a local doctor and turn it into the WMS office or online through Final Forms.
-
For a student to participate in sports they must have good academic standing at school. This means they must have no “F” grades in any classes in order to participate in games. Students can still participate in practices and can become eligible again when weekly grade checks are performed.
Regarding attendance and athletic participation, athletes must attend at least half a day of school to be able to participate in practices or games, regardless of whether absence is excused or unexcused.
Students/Athletes that are currently out on out-of-school suspension cannot participate in practice or games any days during said suspension.
Final Forms
We are very excited to announce that Wapato School District has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic forms for your students. The most exciting news is that FinalForms saves data year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time.
You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update.
All current parents in the district will receive a confirmation email from FinalForms.
Please review the Parent Playbook to get started in FinalForms. If you require any support during the process, scroll to the page bottom-right and click “Get Help”.
If you did not receive an email from FinalForms, please reach out to your school administrator.
